🛣 What Is the 50-Hour Affidavit?

In Ohio, before you can take your driving test and earn your driver’s license, you are required to complete at least 50 hours of supervised driving practice, including at least 10 hours at night. Once you’ve done that, the form needs sign by an eligible adult (one who has witnessed the hours driven) and notarized—confirming that you completed those hours. Ohio Laws+1


You can also use an app to track your hours and receive feedback. The form can then be directly downloaded from the app with all your hours logged. This affidavit is submitted on Form BMV 5791, often called the Fifty Hour Affidavit. You’ll typically turn it in to the Ohio Bureau of Motor Vehicles (BMV) at the time of your driving test or when applying for your license.
